Library Card Application

Individuals who live and work within the Plainview-Old Bethpage Central School District are eligible to receive a free library card. If you live within the district, your card will be mailed to you. In order to renew your account or replace your card you will need to call the library’s Circulation Desk at 516-938-0077, option 6.
